Off Campus Physical Activity

2021-2022  HPMS Off-Campus Physical Activity Information Packet & Application             
Parents & students, please print, read and keep the updated OCPA Information;    
The packet includes the 3-page OCPA Application that will be submitted.

Please note:
An instructor signed Activity Log must be submitted each grading period to the HPMS Teacher of Record. It is the responsibility of the student to complete and submit the participation log.
The three pages of the Application must be submitted together for approval by HPMS.
Completed Application is due May 3rd to Counselor for preferential scheduling. Applications received between May 4th and August 6th will only be approved based upon availability.
