                Parents & students please print, read and keep the updated OCPA Information;    
                Packet includes the 4 page OCPA Application that will be submitted.

           Please note:
          A Participation log must be submitted each grading period
 to the HPMS Teacher of Record.
          It is the responsibility of the student to complete and submit the participation log.
                     

                    The Application is pages 1-4 to be submitted together for approval by HPMS.

            Completed Application (pages 1-4) due May 4th to Counselor for preferential scheduling.
            Applications received between May 5th - August 10th will only be approved based upon
            availability.
