- Extracurricular Code of Conduct Signature Page
- UIL Acknowledgment of Rules
- Concussion Acknowledgement
- Sudden Cardiac Arrest Awareness
- HPISD Emergency Information Form
- Parent/Student Steroid Agreement Form (Middle School students must complete this form as well)
