In order for Highland Park Independent School District to provide the best care for your student with diabetes, please turn in your student's Diabetes Care Plan from their physician to the School Nurse. Listed below is the UDCA (Unlicensed Diabetes Care Assistant) form, which gives the nurse permission to train specific staff members on how to care for your student's diabetes during sporting events or other off-campus activities.
