If you are applying to multiple schools, please complete the Records Request Form for each school.
Private School Applications
- Records requests for private school applications will be submitted to the applying school by the school's deadline.
- Records are only sent directly to the school admissions office via email or the school's submission link.
- Records are not submitted to private schools until after the fall semester report cards are finalized in January.
Recommendation Letters & Evaluations
HPISD does NOT require faculty and staff members to complete recommendation letters or evaluations. Every set of records for private school applications will include a letter from the school's principal explaining the district's practice for recommendations. If a teacher chooses not to complete a recommendation form, the principal letter will be sent so that your student's application will be complete. Below are the MIS and HPMS principal letters for your reference.
If your student is accepted to another school, please complete the Intent to Withdraw form.
